Physicians services are provided by
board certified visiting physicians who will tend to
residents on-site at Mary's every other Wednesday or when a
resident is ill. Families are also encouraged to take a resident to their own PCP whenever they choose.
While Mary's Center for Seniors addresses social issues such as isolation, need for prepared food and manageable living space, there is often an equally important need to address complications of aging and medical syndromes and illnesses that affect our residents. Appropriate management of medical issues may significantly affect quality of life and personal and social function.
Primary care physicians (PCPS) are actively engaged in
Mary's supervision of medications, observations of clinical change and documentation. PCPs make on-site visits and use care planning tools to identify patient-specific risks and care objectives and recommend outcomes. PCP involvement in their patients' care is highly recommended and appropriate care is assured for our residents. The PCP is a key clinical resource for our residents.
Each of our residents on entering
Mary's receives an initial assessment and receives a PCP approved care plan to address their clinical issues.
Mary's staff and residents' primary care physicians collaborate in determining and implementing the patient's plan of care. These plans are patient-centered and accommodate patient preferences whenever possible.
The staff at Mary's inform primary care physicians when they admit one of his/her patients and give the PCP the following information: phone number to the appropriate clinical office, nursing supervisor or administrator and explain the procedure regarding who to contact to communicate new/changed orders or status of resident.
Mary's also notify the physician regarding hospital transfers so that issues such as medication reconciliation and medication management can be reviewed and updated as needed by the PCP.
Mary's believes that the bond between a physician and his/her patient transcends time and place and therefore gives physicians ample detailed information about their patients in order to make appropriate clinical decisions.
